SERVICE PLANS:
Every resident has his/her individual
service plan packet kept on site. This packet is prepared by the Facility Manager, reviewed by the resident
POA or advocated designee, and verified by a Registered Nurse for compliance and accuracy to determine the level of service
for each resident.
When a resident is admitted/acceptance in the facility, he/she will undergo to an observation
phase for a period of 10 days for behavior, responsiveness, and adaptability to ensure a smooth transition to
new living environment is met and satisfied.
NORMAL CONDITIONS:
A review will follow every 180 days (6 months) for Level 1 (Personal Care) and
Level 2 (Supervisory Care) for any updates or changes to the service plan.
Leve 3 (Directed Care) requires a review every 90 days period (3 months) due to individual's
physical, mental, psychological, and pysioloical changes.
NOTE: Any sudden changes or declination of
resident's condition requires an immediate review due to the changes of care to be provided.
